The form helps the SSA obtain information about an applicant's condition, and is the key to obtaining medical records. Now you can get tips right over the Internet on how to best complete the 3368. Just click on any section of the form and you'll get a "plain language" explanation of what the SSA is looking for, why they need the information, and how your answers help them decide if you can get disability benefits.
